Volunteer Declaration

As a volunteer with the St. Joseph Freinademetz SVD – Fu Shen Fu Foundation (the Foundation), I understand that I will be volunteering, either directly or indirectly, with children and other people who are under the care of the Foundation (the clients of the Foundation). I understand that my compliance with all of the requirements below is mandatory for me to volunteer with the Foundation.

1. The references I provide may be contacted by telephone or email.
2. I give the Foundation my permission to use my name and photographs and/or video recordings of me to promote the organization.
3. I will strictly follow all the rules and regulations of the Foundation.
4. I will not engage in any form of political activities during my work as a volunteer with the Foundation.
5. I will not undertake any other job outside of the Foundation for the purpose of earning income.
6. I declare that I am not addicted to drugs, alcohol, or any other kind of substance. I will not engage myself in a distribution or any other kind of trade of drugs, alcohol, or any other kind of substance.
7. I will inform the Foundation of any previous injuries that may affect my ability to safely complete volunteer tasks.
8. I will carry my own health and travel insurance. I will not hold the Foundation responsible for any unforeseen injuries or problems that may occur on the job.
9. I will not initiate or engage in any media/public event pertaining to the activities of the Foundation without the approval of the Foundation Staff or Volunteer Supervisor. Any requests for media engagements will be referred directly to the Foundation Staff or Volunteer Supervisor.
10. I will not abuse, neglect, exploit, coerce, manipulate, retaliate against or deny food or any other basic necessities to the children and other people under the care of the Foundation.
11. I will report to the Foundation staff or Volunteer Supervisor about any incident, action or circumstance which I may become aware of, that presents a threat or endangerment, or poses a current or future impact on the children and people under the care of the Foundation. I understand that it is especially important to inform the Staff or Volunteer Supervisor in the case of a medical emergency, in the case of a pertinent medical update, or in the case of a client’s harmful threat to self or others.
12. I will not disclose to an outside party in a written, verbal, or any other form any personal information about the children and other people under the care of the Foundation, especially their addresses and contact information. I will not transfer such information in an electronic communication such as email, nor will I post such information at a website with an open access to the public, etc.
13. I will not use for personal gain any information about the children and other people under the care of the Foundation, or any information about people and/or institutions working with the Foundation.
14. I will respect the clients' of the Foundation wish to not to take their photos or video recordings, or to not provide any personal information for either personal or security reasons.
15. I will treat all children and other people under the care of the Foundation with dignity, respect and consideration, and I will not discriminate against them because of their race, national origin, religion, gender, sexual orientation, age, disability or marital status.
16. I agree to be sent back home at my own expense if I don't follow the terms of this declaration and/or the rules and regulations of the Foundation.
17. As a condition to receiving a volunteer visa to Ecuador, immediately after my arrival in Ecuador I will sign a declaration to not receive any remuneration for the work with the Foundation.
18. I understand that the terms listed above are not all-inclusive and may be updated, as needed.
